New documents required for driver licenses

Effective Dec. 31, 2009, to apply for or renew your driver license or identification card you will need to provide additional documents.

You must have one of the following to prove identity, date of birth and lawful status: certified U.S. birth certificate; valid unexpired U.S. passport; Certificate of Naturalization; Certificate of Citizenship; valid unexpired permanent resident card; valid unexpired employment authorization document; or foreign passport with valid unexpired U.S. Visa with I-94.

If your name is different than the name on your identity document you will need to bring additional proof of your legal name. Acceptable documents for proof of legal name change are: marriage certificate (issued by a state vital records agency); certified adoption document; or certified court order authorizing a name change (such as a divorce decree.
You must have one document proving Social Security number, including: Social Security card; W-2 Form; SSA 1099 Form; non-SSA 1099 Form; or pay stub (must include name and social security number).

In addition, you must provide two documents proving your principal address that have been issued in the last 90 days: utility bill; rent receipt; phone bill; transcript or report card from an accredited school; bank statement; pay stub or earnings statement; credit card statement. The following items must contain their current address and have been issued within the past year: mortgage document, tax document, court order or homeowners/renter's insurance policy.
